Please bring:

Proof of income from the last 12 months such as:

  1. Tax Return, W-2, letter from SRS, or latest check stub
  2. Child’s Birth Certificate
  3. Child’s Social Security Card (optional)
  4. Medical Card, KanCare, or Private Insurance (if applicable)
  5. Child’s Current Physical/Dr’s note is required for any allergies
  6. Hemoglobin
  7. Blood Lead Result
  8. Child's Immunization Record
  9. 2 Emergency Contacts with name, address and phone #

Currently enrolled students that attend Dodge City Public Schools on an uninterrupted basis will be allowed to continue enrollment. (Based on BOE Policy JBC.)

Nonresident student enrollment information will become available on May 1st, 2024. The number of open seats available to nonresident students for each grade level, as well as the application for those open seats, will only be available June 1st - June 30th, 2024. Nonresident student applications that are accepted will be contacted on or before July 15th, 2024.
