Media Release Form (Promotional Use)

For valuable consideration received, I hereby grant to Dodge City Public Schools (USD 443) (“District”) and its authorized third-party vendor(s):

as well as their legal representatives, successors, assigns, and those acting with their authority, the absolute and unrestricted right and permission to:

• Capture & Use: Photograph, video record, audio record, or otherwise capture my image, likeness, voice, name, and biographical information.
• Publish & Distribute: Reproduce, edit, display, broadcast, publish, and distribute these materials in whole or in part, individually or in conjunction with other photographs, text, or graphics.
• Media Channels: Use these materials across any and all media formats now known or hereafter developed, including but not limited to:
  • Digital & Online: Official school district and vendor websites, landing pages, mobile apps, email newsletters, digital advertisements, banner ads, and electronic communication channels.
  • Social Media Platforms: Instagram, Facebook, LinkedIn, YouTube, X (formerly Twitter), Pinterest, and any other social network or video-sharing platform managed by the District or Vendor.
  • Print & Physical Media: District newsletters, brochures, flyers, posters, school yearbooks, event programs, press releases, local news features, direct mail, and display boards.
  • Broadcast & Video: Local television, district broadcasts, webcasts, podcasts, promotional videos, and educational or recruitment materials.

I understand that these materials will be used for educational, promotional, marketing, public relations, advertising, and informational purposes related to Dodge City Public Schools, its programs, activities, and designated third-party vendor projects.

• I agree that all photographs, audio recordings, video footage, and digital files created by or for Dodge City Public Schools and the named third-party vendor are and shall remain the exclusive property of the District and/or Vendor.
• I acknowledge and agree that I will receive no monetary compensation, royalties, or further considerations now or in the future for the use of my image or likeness as authorized by this release.

I waive any right to inspect or approve the finished product, advertising copy, digital edits, or other printed/electronic matter that may be used in connection with my likeness, or the specific use to which it may be applied.

I hereby release, discharge, and agree to hold harmless Dodge City Public Schools, the named third-party vendor, and their respective board members, officers, employees, agents, licensees, and assigns from any and all claims, demands, or causes of action arising out of or in connection with the use of the materials, including without limitation any claims for defamation, invasion of privacy, right of publicity, or copyright infringement.
